I sent my EC application with all the supporting document on Friday 11/02/2007 and received my EC approval today 11/06/2007. I requested a post dated visa and it was granted starting in February 2008.
Here is the list of supporting documents (I completed the application and made the payment online. PS: application are only accepted by mail in the Los Angeles office - do not take into account the fact that the link below give the option to make an appointment online; as a matter of fact the link doesn't work):
* completed application form VAF1 (completed online)
* your current passport
* your old passport or a copy of it
* Immigration status (Original not copy):e.g. Permanent residence card,
Valid I-94 H1-B - L-1 - B-1 I-20 (Signed on reverse side) IAP-66
Employment authorisation card with advance parole document
* evidence of funds available to you, in the form of bank statements
covering at least the last three months
* either a letter confirming your period of leave from your employer
together with salary slips for the last three months or a certificate
confirming you have continuing studies or if you are self employed,
evidence of your business.
* a valid UK work permit issued by Work Permits UK or for HSMP your
valid approval letter
* If you wish to attend our office in person you must have a confirmed
appointment. You can book an appointment online at:

* One recent colour passport photograph (not more than six months old)
Please note: scanned or copies of photos are not acceptable
NOTE: - 2 colour passport photos are required instead of 1
- Also I didn't have to send the letter of resignation from my employer.
- For the evidence of funds, I sent my bank statements and the Kelly Blue Book current value of my car

I wish i would have seen your post before i sent mine...
I just sent out my application last night, so it may have reached at LA consulate today. I wasn't aware of 2 photos but at the last moment i threw another photo (but both photos are different from each other) in an application envelop. I sent my 4 paychecks, employment verification letter, ALL I-XXX receipts/approvals, EAD card (original) but not AP document (technically it may not required since you are planning to live and work in UK and not returning back), last year tax return, 3 months bank statement, 401k statement and of-course HSMP approval letter.
It is good to hear that they consider the requests and grant the post dated visa, I've requested mine in March'08, let's see how it goes...keeping my finger crossed )(

Whatz’up ? From my experience, the EC process at the LA office is pretty straight forward.
- I did include 4 extra photos in my application just in case (they were all returned to me along with other supporting documents).
- Yes I did receive an email to acknowledge the receipt of my application
- Also I received an email to let me know that my passport with the visa and all supporting documents were being returned to me via UPS overnight.

I forgot to mention that I asked them in my cover letter to acknowledge the receipt of my application and to give me any update via email.
BTW: they can only post date you visa up to 3 months max according to their law. i.e. if you want visa starting in March 08, you should send your application in December 07
