Do we need to sumit all the documents when we reapply after refusal? or we just resubmit forms & fees only?

New application is new application and will bear no history of previous one, other than a record of their end. But in the form (cant remember which one) where it asks "have you ever refused entry to any country", you need to declare it with explanation of what happened.
