Topic: Help needed with CAIPS received
Hi everyone,
I am sponsoring my wife and I am going though the Paris office. I recieved my CAIPS recently and I have a few questions:

Now the questions:
1) The IO from CPCM indicated in the notes the following:
"Please note no info on back of photos and additional family information from section 'B' not signed. Therefore, service commitment date not entered. CPCM/AO. Case referred to Paris"
Now, I submitted about 12 pictures of the wedding and our honeymoon. I do not know if this info on back of photos means what! what kind of info is the IO wondering about! It is a picture which is better than a thousand words. but anyways, I looked at the addition family information sheet which I supplied with my application and section 'B' (for dependent children info, which doesn’t apply in our case sinc no kids) is actually SIGNED! and it is section 'A' that is not signed since Note 1 in that section indicates that "If no spouse or common-law partner is listed in section A, read and sign below", so in this case my wife did not sign below since my name (spouse) is listed in that section!
Do you think I should contact CIC and clarify this issue with them? It seems from the CAIPS that they have not requested any documents anyways! so maybe I should not bother at all?
2)What does it mean that “the service commitment date not entered”?
3) What do the “0s” in front of the codes represent?
4) What does the “1” in front of the Sec Crim mean? Should I say that they finished the background check and it is now in the process of issuing the visa?
5) What is the “&” supposed to mean in the following:

6) It seems that the file went through 4 IO and went back “BF’D” to AB on 22 July 2008. Now do you think this is the final stage of process? Would you think AB is the guy to close the deal?
Overall, would you say that things are smooth and I should not worry much and hopefully expect my wife to get a letter with the PPR request?

Pass
Crim checks and background checks aren't always the same..... I can't offer more info as I filed inland but I would google "CAIPS DECODED" for more info.

can you tell me please who is the adress where you sent yours application for CAIPS?
My husband who live in Toronto want to get it, but he don't know where to sent the application for Caips.

Make out a cheque payable to the "Receiver General of Canada" for $5.00
Enclose the cheque in an envelope with the other forms, and a copy of your husband's Canadian ID (or a passport copy) and send it to the address below:

i need your help about the compilation of CAIPS.Any part of document must be completed with data from my husband? In space "requester's reference" I put the ID of my husband? My data must be written only in the space of Caips information and in wich embassy?

Hi bruna,
I had instructions from someone on a forum, how to do it. However I will describe the complete details so that you may not have any more doubt.
Please make sure that there are form for you and for your husband seperately. Follow the instructions below:
1. fill out this form:

Step by step instructions:
Tick the box that says "I authorize Citizenship and Immigration Canada... ..., to release information from my case file to the following individual."
The fill out all particulars (name, DOB, location of VISA Processing centre, i.e. The Canadian Embassy in .............
Then fill out your husbands personal information under the headings titled "Designated Individual."
sign AND date the form at the bottom
OR, if you have already filled out this form:

then just send a photocopy of it along with 2. and 3.
2. You must now fill out this form:

Step by step instructions:
Leave the first two boxes at the top blank (Requesters Reference No. and Public Rights Administration file number).
Tick the box that says "Access to Information Act ($ 5.00 fee)."
You HAVE to pay $5.00, there is no way around this.
Do NOT tick "Privacy Act" and hope they will just process it without you having to pay; this will NOT work. You must pay $5.00 since you are requesting the info on behalf of your spouse, and your spouse is not residing in Canada.
Select which language you wish to receive your notes in, English or French. (These choices are under the "Privacy Act" sub-heading. ).
Then tick the box which says "Receive copies of the Records."
Fill out your spouse's information under the "Client Information" box.
Under the next heading, "Request for-Required Information" box, tick the "CAIPS (information abroad)" box and write "The Canadian Embassy........." under the "Embassy" sub-heading, and write the "Visa File No." in the appropriate box.
DO NOT check any other boxes in this section. Especially the "OVERSEAS VISA FILE" box. If you do this, they will pull your file aside, photocopy everything and send it to you.
It will not only take you forever to get the information, but they will also take your file out of the queue to process your request and therefore it will take LONGER !!! (CAIPS is just a computer printout of all the work done on your file so far. It will not affect the speed of your file processing.
In the next heading, tick the box which says "This request is being made by a Canadian Citizen..." Fill out all of your information in this box, name, phone number, address, etc.
Sign and date.
3. Make out a cheque payable to the "Receiver General of Canada" for $5.00
Enclose the cheque in an envelope with the other forms, and a copy of your Canadian ID and send it to the address below:

I have already replied but you may get confused as I mentioned "you" in some place and "spouce" thinking that you are in Canada and your husband is outside. so just reverse wherever applicable.
Form 5563 is for your husband in Canada. .
He should sign that form.
I think it is clear for you now.
