FLR -HSMP - Applying from UK
FLR form is same as Tier1 form now.

It is on the right side of the page http://www.bia.homeoffice.gov.uk/workingintheuk/pointsbasedsystem/applying/initialapplications/
with name 'Application for initial entry (switching) or extension of stay within Tier 1 (General) - main applicant'

I hope you have valid HSMP approval letter.
