| South indian name with initial & Mother's name not present on BC - did anybody face problem ? |

Does not state the reason in as such. I am guessing that my mother's name absent on the BC is the reason.
It only talks about getting hospital/school/census/baptisum records for evidence. They are also not talking about any affidavit !
No need to panic; this is a common situation with mother's name missing from BC. My advice will be as follows:
1. Get at least two affidavits from your parents/relatives who can testify under oath about your parentage and date of birth. These individuals have to be close enough to you in relationship to make this claim AND old enough to have known you when you were born.
2. No need to send the ration card if it does not have the right birth date; it is only going to confuse USCIS!
3. You can still send the hospital discharge letter even if it does not have your name. You can also try to contact the hospital and request them to add your name. It might be worth a try.
4. Send in your high school/10th grade certificate. These usually have date of birth and father's name.
