Posted by Hello raj20092k, did you check if your employer has received the questionnaire from DOL after submitting the case to DOL. Employer will receive 3 e-mails after submission. First email that gives you the case number A-xxx-xxxxx. 2nd e-mail is the questionnaire, and the employer has to complete the questionnaire and return it to DOL within 3 days of receipt. After DOL receives the updated questionnaire, then 3rd e-mail will be received that says the application submission is successful. My employer did not receive the questionnaire, I am just wondering if your employer has received it or not?
