How and when did you find out it was lost ?

Didn't find out until I asked my lawyer why I didn't get any response while all other employees who filed at the same time already had their confirmation. Turns out that we never even received a verification e-mail for my application (after months). So we filed again and received a verification after a day. I'm angry at my lawyer for not mentioning that we didn't get a verification email which apparently seems to happen very fast.
