Notes:

I was on H4 and had applied for COS to H1. Got selected in lottery. I got a new H4 on 25 Apr 2014 when my spouse changed job. Old H4 was revoked on May 10, 2014. I am guessing my RFE is related to the above condition. Will update when I know more.
