US CIS wants the approval notice for all L and H visas that I have held for the past 6 years so I had to submit all of that. I initially submitted the approval notice only for the latest (active) visa that I currently hold.

In Nov 2009, Approval Notice sent by USCIS, dated July 24,2009. As of Oct 12, 2009 still waiting for decision. Employer said visa has been approved but still wating for the Approval Notice.
