RFE was about:

1: Proof of lagal stay in US from AUg 2001 to Jan 2004 ............ Was in school so provided all the I-20 along with transcript and attendance and verification letter from school

2: Public charge issue because I mistakely answered the question yes ............ Send the IO a detail explaination about the mistake

3: Medical test to be taken again............... did not do it before so I have to do it again.

yes i did , kinda more knowledgeable in this matter, since I am a seasoned player and visiting all this immigration forums for quite some years. Secondly it was a straigh forward RFE.

The address you requested is below, dont forget to place the orange paper on top of all the documents and then a copy of the RFE mail and then you rfe response document, its a rigid rule if u dont follow that way then USCIS will surely mess up things.

Did you had two approved I-485s?

Secondly, in one of your above posts about RFE you've ssaid "3: Medical test to be taken again............... did not do it before so I have to do it again.". Can you please elaborate. Didn't you send the medical reports with 485 application?

No one is mine and the other is the dependent

Applicant Type: dependent .

I did send the medical along with the I485 but they send me a RFE for my spouse to redo the medical. So I had to resubmit the medical. Since it was the most simple RFE to tackle i did not ask any questions and went for it.
